Next Step - Beginning Care

Great - you've attended our Meet the Midwife and Tour (or are a returning client), had time to discuss it with your partner and decided that APM is the right fit for you. So what's next?


Once you've made the decision, we ask that you contact our office and speak with our billing specialist, Deanna Marrujo at (909) 464-0974. Deanna can help you with payment arrangements, get you scheduled and help you with paperwork. If you feel you might have something that would exclude you from care, we can do a preliminary RISK assessment to determine if you're low-risk.  


If you have a PPO insurance, begin the Larsen Billing Service verification of benefits process; Insurance Verification Form on Larsen Billing Service's website (the APM code is 07091).


If you've been seeing another care provider and will be transferring care, you can complete a Medical Records Release and send it to your previous doctor or midwife. We'll need your labs, ultrasound reports and prenatal records. If you're a VBAC mama, we'll need your prior operation report from the c-section and your previous pregnancy medical records. 


We'll set you up for your first appointment and get you the necessary forms to fill out before your appointment. We're thrilled to be a part of your journey!   

Client Portal

APM uses a private and secure computer-based Health Record.  


Once you've scheduled your initial visit you will be sent a link where you can enter all of your registration and health history from the comfort of your home PC or MAC, I-Pad or I-Phone. 

You will get the chance to read and review all of our Informed Consents and Financial Agreement.  Those with either an I-Pad or I-Phone have the opportunity to sign these documents from home. 

At your first visit you can complete the process by taking your photo and providing verification of your insurance cards and drivers license, and reviewing for accuracy.  

What to expect the first visit

At this first visit we'll be doing several things including:

  • Reviewing your APM Health Record
  • Going over paperwork (Informed Consents, Financial Agreements and payment arrangements, Insurance, MANA stats, etc)
  • Detailed Medical History
  • Physical exam
  • Well woman visit (if needed)
  • Laboratory testing (if needed)
  • Creating a plan for your healthy pregnancy

Please allow 1 to 1 1/2 hours for this depending on your needs. 


Creating your APM Health Record

At least 72 hours prior to your initial visit, complete the client registration using our client portal.  This gives the midwife time to review your history.  When you come in for your first visit you can complete the process by taking your photo and providing verification of your insurance cards and drivers license.


Going over paperwork (Informed Consents, Financial Agreements and payment arrangements, Insurance, MANA stats, etc).   Deanna or one of our staff will go over all these forms with you. This should take about 30 minutes. 


Detailed Medical History

Our midwifery staff will review your health and medical history with you, get clarifications and answer questions. Review can take 20-40 minutes depending on your history.  


Physical exam

This midwife will do a complete head-to-toe physical exam; checking blood pressure, listening to your heart and lungs, and assessing everything from your thyroid to your back and hips (first steps in our Spinning Babies and Posterior Baby Prevention!). We'll listen for a baby heartbeat if you're far enough along. This usually takes 15 minutes


Well woman visit

Our midwife will do a pelvic and breast exam, obtain a genital culture and do a pap smear if needed. If these things have already been done by a previous provider, they will not be repeated. Add 15 minutes. 


Laboratory testing (if needed)

If this is your first pregnancy visit, we'll need to draw blood for a prenatal pane and get a urine sample. Be sure to drink plenty of fluids prior to your appointment to make this easier for you. Allow an additional 10 minutes for lab work. 


Creating a plan for your healthy pregnancy

You'll leave with loads of information about your pregnancy including supplements, lab and ultrasound, nutrition, exercise , childbirth classes, massage therapy and stress reduction. Our goal is to help keep you as healthy and low-risk as possible! Allow 5-10 minutes.
