Great - you've had a chance to speak with one of our midwives or attended our Meet the Midwife and Tour (or are a returning client), had time to discuss it with your partner and decided that APM is the right fit for you. So what's next?
Once you've made the decision, we ask that you contact our office at
(909) 464-0974. We can help you with payment arrangements, get you scheduled and help you with paperwork. You'll be sent a link so you can create your chart from any home device. Once you have created your chart we set up a time to do a History Review and RISK assessment. This is done via a Telehealth appointment and takes about 45 minutes as we review all of your medical history and your pregnancy concerns with you. There is also a Password Protected area for clients where you can watch our two short welcome videos where we review things such as how and when to call the midwives and reviewing the Informed Consent packets.
If you have a PPO insurance, APM can assist you in beginning the verification of benefits process. See below for the Verification Of Benefits form.
If you've been seeing another care provider and will be transferring care, you can complete a Medical Records Release and send it to your previous doctor or midwife. We'll need your labs, ultrasound reports and prenatal records. If you're a VBAC mama, we'll need your prior operation report from the c-section and your previous pregnancy medical records.
We'll set you up for your first appointment and get you the necessary forms to fill out before your appointment. We're thrilled to be a part of your journey!
